Making a Referral: Anyone may refer a patient to Compass Hospice. The patient may refer
him/herself, as well as a friend, caregiver, relative, social worker, nurse, hospital, nursing home, another Home Health Care
Agency, or the patient's private physician. Our staff is available to visit with the patient and family prior to admission
into Compass Hospice program for assessment and explanation of the services provided.
To make a referral: call (940)627-2888 and ask for the Referral/Admissions
Desk. The following is an example of the information that you will be asked for:
- Patient's name
- Patient's phone number
- Patient's physician's name
- The name of the person that is referring and their phone number
The
following information is helpful, but not absolutely necessary at initial call:
- Patient's address
- Patient's date of birth
- Patient's Social Security number
- Insurance information and/or Medicare or Medicaid
- Patient's diagnosis
- If possible, the name of the person that will be the caregiver
At
this time the Referral/Admissions Staff will contact the patient's private physician to obtain consent to accept the patient
into Hospice and will also obtain the remainder of the information that is necessary to complete the referral.
Admission Criteria for Life-Limiting Illness:
- Patient's private physician must state that the patient has six months or less to live, when treatment is
palliative and not to cure the disease.
- If the patient's private physician will continue to care for the patient while under the services of Hospice.
- Patient must have someone that will care for them 24 hours a day when the need arises.
- Patient must meet admission criteria established by Health Care Financing Administration.
